Improving health outcomes and strengthening community partnerships.

GEORGIA CEAL RESTORES
Georgia Community Engaged Alliance (CEAL)-Reimagined Environments and Systems Together for Outcomes through Research and Evaluation for Sustainability (RESTORES) – GA CEAL RESTORES was designed to prevent chronic disease development, address mental health and advance social determinants of health (SDOH) in communities through community-based participatory research (CBPR). This approach will adapt, culturally tailor, and implement the nationally recognized and evidence-based Diabetes Prevention Program (DPP) with families.
Community Informed Solutions
The Georgia CEAL RESTORES initiative is designed to positively advance community-level SDOH regarding diabetes prevention and other health issues through training, capacity building, and mobilization (i.e., policy, organizational/community levels). To address the problems in these communities, we propose a pilot study in four counties: Bibb, Houston, DeKalb and Fulton.
